Molteno3

The new Molteno3® has a larger, thinner, more flexible plate than earlier designs, shaped to fit the curvature of the eyeball and with an oval subsidiary ridge on its upper surface. This design allows the implant to sit snugly between and slightly beneath the adjacent extraocular muscles. When covered by Tenon’s tissue the subsidiary ridge of the implant and the overlying Tenon’s tissue together form a self-cleaning ‘biological valve’ which cannot become blocked by exudate or blood clot.
This valve reduces postoperative hypotony by restricting the drainage of aqueous to the small primary drainage area until the IOP rises sufficiently to lift the tissues and allow drainage of aqueous over the entire plate. This modification results in a thinner bleb capsule, a lower final IOP and less need for postoperative hypotensive medication.
The new Molteno3® implants have undergone rigorous testing since early 2004. Results have been impressive with 2/3 of patients requiring no hypotensive medication and with a 95% success rate in most glaucomas.
The results of all the first 71 Molteno3® non-neovascular cases were compared to all 134 non-neovascular cases since 1995 where a double plate Molteno3® implant was inserted. The results are summarized in the graph below and show that the IOP of eyes receiving the Molteno3® is consistently lower than in those with the earlier design.
